Overview of HIPAA* Privacy Practices Notice
Federal law requires OSF Healthcare System and its related health care providers and health plans to maintain the privacy of individually identifiable health information and to provide you with notice of their legal duties and privacy practices with respect to such information. OSF Healthcare System and its related health care providers and health plans must abide by the terms and conditions of this Privacy Notice.

The health care providers participating in OSF are required to seek your written acknowledgment that you have received this Notice. By furnishing written acknowledgment of receipt, you do NOT indicate your agreement or consent to the uses and disclosures of information described in this Notice. The acknowledgment indicates only that you have received this Notice. You may decline to furnish written acknowledgment of receipt. In this event, your refusal will be documented.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU WILL BE PROTECTED, HOW IT MAY BE USED AND DISCLOSED, AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

The public has a right to access information about our nurse staffing. Nurse staffing includes the number of registered professional nurses, licensed practical nurses and other nursing personnel assigned to each patient unit each day.   To receive this information while you are or your loved one is hospitalized, contact the health unit coordinator (HUC) on the specific nursing unit.  Your request will be answered by a nursing supervisor or manager.  You may also call the nursing services office at 1/815/395-5195, or put your request in writing to: Nursing Services, OSF Saint Anthony Medical Center, 5666 E.
